Coverage requirements:
- Have a prescription dated less than 5 years ago
- Have not received glasses reimbursement during the current calendar year (check your health insurance policy)
All health insurance plans are eligible.
You'll receive by email the reimbursement form to send to social security and the paid invoice to send to your health insurance provider.
