Insurance coverage
Eligibility requirements:
- Have a prescription dated less than 5 years ago
- Have not received glasses reimbursement during the current calendar year (check your insurance plan)
All insurance plans are eligible.
You'll receive by email the claim form to send to Social Security and the paid invoice to send to your insurer.
