Insurance coverage
Eligibility requirements:
- Have a prescription dated less than 5 years ago
- Have not received glasses reimbursement during the current calendar year (check your insurance plan)
All insurance plans are eligible.
You'll receive by email the claim form to send to Social Security and the paid invoice to send to your insurer.

Category 3 lenses with 100% UV protection and a light blue gradient tint. Can be made to your prescription: send yours after ordering and our opticians take care of the lenses.
