- Have a prescription dated less than 5 years ago
- Have not received glasses reimbursement during the current calendar year (depending on your health insurance plan, please check)
All health insurance plans are eligible.
You will receive by email the claim form to send to Social Security and the paid invoice to send to your health insurance provider.
