Insurance coverage
Eligibility requirements:
- Have a prescription dated less than 5 years ago
- Have not received glasses reimbursement during the current calendar year (check your insurance plan)
All insurance plans are eligible.
You'll receive by email the claim form to send to Social Security and the paid invoice to send to your insurer.

Jazmine Black & Tortoise: oversized two-tone acetate sunglasses
A thick, oversized black acetate frame with tortoiseshell interior visible along the temples. Rosy brown gradient tinted lenses. Works beautifully on oval, square and long face shapes.
Protection and lenses
Category 3 lenses (100% UV protection), rosy brown gradient tint. Can be made to your prescription: send yours after ordering and our opticians take care of the lenses.
