- Have a prescription dated less than 5 years ago
- Have not received glasses reimbursement during the current calendar year (check your health insurance policy)
All health insurance plans are eligible.
You'll receive by email the reimbursement form to send to social security and the paid invoice to send to your health insurance provider.

Category 3 lenses with 100% UV protection, uniform rosy-brown tint. Can be made to your prescription: send us your prescription after ordering and our opticians will take care of the glazing.
